About the Role

We are seeking an experienced health professional. Who will be responsible for the operational delivery of the National Framework for NHS Continuing Health Care (NHS CHC), Funded Nursing Care (FNC) and implementation of NHS CHC Personal Health Budgets (CHC PHB).

The role involves:

  • Managing and leading the CAPT service and its functions, including integrated and jointly funded services.
  • Ensuring assessment, case management, and review of all Health-funded Complex cases align with National Guidance.
  • Maintaining service flexibility to meet changing requirements.
  • Organising events and meetings, and optimising communication tools for effective dissemination of information.
  • Identifying and procuring products, equipment, services, and facilities while considering budget constraints.
  • Overseeing team recruitment and recruitment panels.
  • Conducting appraisals, clinical supervision, personal development, and addressing disciplinary or capability issues.
  • Ensuring compliance with statutory and mandatory training for self and the team.
  • Supporting, motivating, and developing staff to align with NHS strategy.
  • Collecting qualitative and quantitative data, contributing to business cases and project outcomes.
  • Analysing and presenting data for decision-making and risk assessment.
  • Proposing, drafting, and interpreting policy changes and service-level agreements.
  • Maintaining awareness of emerging government policies to inform service strategy.
  • Facilitating professional registration revalidation.
  • Assisting with new staff training and induction.
  • Undertaking directed research and development activities.
  • Managing clinical aspects of inquiries, disputes, challenges, and complaints in accordance with policy.
  • Leading project teams, making decisions in the project's best interest.
  • Serving as a point of contact and case manager for individual cases.
  • Managing complaints and queries related to the team's caseload.
  • Providing accurate assessments to identify appropriate services, avoiding hospital admissions, and adjusting care plans.
  • Ensuring high-quality work delivery within budget constraints.
  • Adhering to budget and expenditure monitoring, working with the Business Manager.
  • Contributing to performance and governance strategies and improvement programs.
  • Handling sensitive information and presenting it to stakeholders.
  • Developing short, medium, and long-term business plans for quality outcomes.
  • Building positive relationships with internal and external stakeholders.
  • Resolving conflicts and contentious situations professionally.
  • Aligning services with NHS strategic priorities and making recommendations to senior management.
